Sending Patients a Secure Link to Complete Health Forms

Overview

To help collect required patient forms before appointments, you can now send patients a secure, one-time link that gives them direct access to their questionnaires, with no patient portal login required.

Details

When a patient follows the secure link, they are taken directly to their Questionnaires page in the patient portal. From there they can view, complete, and submit any forms visible to them, based on your questionnaire display rules. They will not have access to any other area of the patient portal unless they choose to log in.

If a patient has completed all applicable forms, the page will display: "There are no additional forms to complete at this time."

Sending the Link from a Patient's Chart

1. Open the patient's chart and navigate to the Patient Portal menu.
2. Click Send Questionnaires Link (located below Questionnaire Manager).
3. An email composition window will open with the subject line and message body pre-populated. You can edit the message before sending, or send it as-is.

The pre-populated email includes a personalized greeting, a brief explanation of why forms are being requested, and the secure link for the patient to access their forms.

Security & Link Expiration

Each secure link is single-use. If a patient returns and clicks the link again later on (after their initial session expires), they will see a message letting them know the link has already been used and a new secure link will be automatically sent to their email address.

What Patients See

• All questionnaires currently applicable to them, based on your Questionnaire Manager display rules.
• Previously completed questionnaires, along with their completion date, unless those are set to disappear once they are completed.
• An option to log into the full patient portal if they wish.

Forms submitted via the secure link will appear in the Patient Portal Queue assigned to the correct patient, just as they would if submitted through a standard portal login.
