The Questionnaire Status Report helps you track questionnaire completion and manage patient follow-up effectively.

Ways to Run the Report

You can run the Questionnaire Status Report from:

  • Scheduling menu - Run the report for patients with appointments in a given period of time.
  • Patient Portal menu - Run the report for an individual patient from their chart.
  • Advanced Patient Search - Run the report for a custom list of patients (use Send Results to Report).

When running the report for patients with appointments in a date range, you can filter by:

  • Provider
  • Location
  • Appointment type
  • Appointment status

Understanding the Report Layout

In the report itself, you will see the list of patients. Each patient entry includes:

  1. Patient Details - Name (clickable to open their chart), DOB, phone number, email address. 
  2. Portal Access Details - whether they have access and last login date/ time.
  3. Questionnaires - which questionnaires are visible to that patient and completion status. 
  4. Appointments - All appointments in the reporting period, or their next 3 upcoming appointments.

Which Questionnaires are Shown

Questionnaires displayed in the report follow your Patient Portal display rules (Patient Portal > Questionnaire Manager).

  • Completed questionnaires are listed on the report even if they wouldn't be shown for that patient on the portal based solely on the Date of Last Completion. This enables you to see which questionnaires are completed, even if you have those disappear from the portal. 
  • Questionnaires marked as "nudged" appear at the top of the list.

Which Appointments are Shown

The report includes:

  • All of the patient's appointments within the reporting period (if run for a date range.
  • The patient's next three upcoming appointments (if run for a specific patient or patient list).

Click on any appointment to edit its status, add notes, or cancel.
