Overview
The patient's most recent blood pressure (BP), weight, and height readings are shown in the BP, Weight, Height block on the patient dashboard or encounter note page. Hovering over an entry will show a larger view, with details about who entered the most recent measurement. Clicking on BP, Wt, or Ht will allow you to add a new entry. Vitals are a critical component of patient care documentation and can be tracked over time to monitor trends and health changes.

Where to Find Vitals
The Vitals block is displayed on both the patient dashboard and within encounter notes. On the patient dashboard, it appears as a summary widget showing the most recent readings. Within an encounter note, vitals are included as part of the clinical documentation. You can also view a patient's complete vitals history by expanding the Vitals block using the pop-out icon.
Add Vitals
To add a new blood pressure, weight, or height measurement, click on BP, Wt, or Ht in the Vitals block. If you have custom vitals listed you can similarly click on these listings to add entries. Enter your measurement in the pop-up that appears and if the date of reading is not the date you are entering the data make sure you adjust the date of the reading field by using the calendar option. You may also enter notes about the vitals reading for patient weight or height. Click Save.
Editing and Deleting Vital Entries
Once vitals have been entered you cannot edit them, but you can remove errant entries. To do so just pop out the vitals block using the arrows icon to see the full vitals history. Find the entry you want to remove and click the delete (X) icon next to it. A confirmation dialog will appear before the entry is permanently removed.
Viewing Vitals History and Trends
To view a patient's complete vitals history, click the pop-out icon on the Vitals block. This opens a full view showing all recorded measurements with dates, values, and the staff member who entered them. You can use this view to track trends over time, such as weight changes or blood pressure patterns. The vitals history is sorted chronologically, with the most recent readings at the top.
Custom Vitals
In addition to the standard BP, weight, and height fields, Cerbo supports custom vitals that your practice can configure to track additional measurements. Common examples include BMI, waist circumference, temperature, pulse, respiratory rate, and oxygen saturation. Custom vitals appear in the Vitals block alongside the standard fields and can be added and tracked the same way. Vitals in Encounter Notes
When creating an encounter note, vitals entered during or before the visit will automatically appear in the encounter documentation. You can also add new vitals directly from within the encounter note. This ensures that all clinical measurements are captured as part of the visit record. If vitals were recorded earlier in the day (for example, by a medical assistant during intake), they will already be populated in the encounter note for the provider to review.
