Your patient portal's Forms & Questionnaires page is your centralized location for completing all required paperwork, from initial intake forms and symptom assessments to consent documents and follow-up visit questionnaires. Access everything you need before or between appointments in one convenient place.

The Forms & Questionnaires page on the Patient/ Client portal is generally where you as the patient or client will complete and submit initial intake paperwork, symptom questionnaires, consent forms/ practice policies, and follow up visit paperwork. Each clinic's Forms & Questionnaire page will look different, and will include different forms and questionnaires for you to complete - as selected/ set by your provider's office.
