Overview
Your Patient Portal is your gateway to your health information, appointment requests, and secure messaging with your provider.
Your username and password were set by you when you registered online or activated your account via an email invitation. Your username may be your email address or another unique identifier.
Sharing an email with a family member? Only one person can use a shared email as their username. We recommend each family member use a distinct username instead (e.g., first + last name) to avoid confusion.
Having trouble getting set up initially? Contact your provider's office directly.

Forgot Your Username or Password?
Click "I forgot my username" or "I forgot my password" on the login page, then enter your confirming information. This must match exactly what your provider has on file:
- Email address on file with your provider
- Date of birth
- Last name
- Zip/postal code (U.S.: first 5 digits)
If your information is verified, you'll receive an email with your username or a password reset link.
I Didn't Receive the Email
Wait 15–20 minutes, then:
- Check your spam/junk folder
- Search your email for do-not-reply@md-hq.com
- Add that address to your safe senders list and try again
Still nothing? Contact your provider's office.
I Got an "Unable to Authorize your Account" Email
This means the information you entered didn't match your provider's records, or your Portal access has been disabled.
Common causes:
- Recently moved → try your old zip code
- Recently married/changed your name → try your old last name
- Some clinics have similar names, so make sure you're on the right patient portal. Your clinic may be using a different patient portal, so reach out to them for assistance.

My Username and Contact Email Are Different
In rare cases, your Patient Portal username may differ from the email address your provider has on file. If this seems to be blocking your username/password request, contact your provider's office.
